Real World Experience with Safety Gear

Greetings all!  I just started riding about 2 months/200-ish miles ago.  I've been lurking on this forum since way before that tho.  It's been an amazingly invaluable source of information.  

Since before I started, the decision about what safety gear to wear has been something I've wrestled with a bit.  Despite not yet having any type of serious crash, I'm pretty paranoid on the subject.  I commute to work about 6 miles daily, mostly on multi-use paths, but about a mile and a half is on public roadways (albeit lesser used roads that are designated bike routes, so not much vehicular traffic to worry about).  All pavement, obviously.  Currently, I'm wearing a full face downhill MTB helmet, hard shell elbow pads, soft d3o knee pads, and wrist guards.  I typically don't exceed around 20-22 MPH, so I guess I'm relatively conservative with speed.  Some of the changes I'm thinking about: switching to hard shell knee pads so I'll have a better ability to "slide" if I go down, adding in some type of jacket like the Demon Flex Force X2, and switching to full knee/shin guards.

What I'm wondering is this: for those of you who have had crashes where you were geared up, what safety gear were you wearing that worked and what were you wearing that did not work?

Specific brands/models are great, what surface you were on, type of crash, speed, etc.

Still looking for a good elbow guard, but I can vouch for these items, no hesitation. I plan to get the Demon armored jacket for winter.
